Our commitment is to provide your loved one with a level of care appropriate to their needs. To determine needs, a professional assessment is made by a Registered Nurse upon admission, re-admission and periodically. Changes in a resident’s needs, behaviors and physical condition are continually observed and reported.
When a resident’s condition changes, a determination is made monthly by Windemere Park’s Registered Nurse regarding the most appropriate level of care, and the billing level is adjusted, either up or down, for the following month.
Windemere Park assures adequate and appropriate care is available to every resident by adhering to the following levels of care definitions:
Independent Plus: Residents at this level are functionally independent or require minimal assistance with mobility, dressing, grooming and bathing and are mentally alert. They may need nutritious meals (including a special diet), assistance with housekeeping and/or laundry and the stimulation or security of not living alone. Medications, if any, are administered by Windemere Park staff. They report any significant changes in their condition, or physician visits to Windemere Park, and agree to an assessment by a Registered Nurse on admission, and at least every six months.
Assisted Care: Residents’ needs at this level are the same as for Level I, yet require additional staff assistance. Medications may require special monitoring, i.e., flood work, frequent changes in physician orders for unstable medical conditions, etc. Routine weekly or semi-weekly assistance is required with one or more activities of daily living such as bathing, grooming, toileting, ambulation, transfers, eating, etc.
Supportive Care: Residents at this level need 24-hour staff availabaility and may need involvement of mental health or other professionals to accommodate their needs. Residents at this level require significant daily assistance with one or more of the following:
Windemere Park’s aging in place philosophy allows many residents, especially those who make Windemere Park their permanent home, to remain at Windemere Park even when their needs increase to Level III or higher. In these situations, home health and/or hospice services from a community agency may be recommended by Windemere Park to work in collaboration with our own staff to meet the resident’s needs.
Whenever potentially unhealthy or unsafe behaviors are observed in a resident, Windemere Park will confer with the resident, the resident’s family, the resident’s physician and its own staff to determine how to best support and respect the individual resident’s autonomy, avoid the trauma of moving if possible and still safeguard the resident as much as possible from undue risk or harm. If the resident, resident’s family, resident’s physician and Windemere Park’s Administrator agree on an acceptable service plan, as documented in a Shared Responsibility Agreement, Windemere Park will assure the resident receives agreed upon services.
